Nurse Practitioner Program
Patients have direct access to the nurse practitioner by telephone, email and video conferencing, day or night.
Outpatient Care Program
This outpatient-only program offers patients direct access to the primary physician, Dr. John R. Diaz, and the nurse practitioner.
Outpatient and Hospital Program
The most comprehensive of our Concierge Medicine Programs, the Outpatient and Hospital Program offers direct email, video conferencing and instant messaging access with the doctor and nurse practitioner, at any time of day or night. The doctor will also be available by telephone. Dr. Diaz maintains hospital privileges at both Naples Community Hospital campuses —NCH (downtown and NCH) and North Collier.
