It also removes barriers of access, providing full communications with Dr. Diaz and his nurse practitioner 24/7. This care is based on knowledge of the patient in the context of the family and the community, emphasizing disease prevention and health promotion. When referral is indicated, the family physician refers the patient to other specialists or caregivers but remains the coordinator of the patient’s total well being. This prevents fragmentation of care in both the outpatient and inpatient settings. Anchor Health Centers provides all Concierge Medicine patients’ full access to Anchor Health laboratories, imaging and radiology departments as well as other Anchor specialists and medical testing departments.
