Once established with your primary care provider, you will be guided through all Anchor specialty and ancillary care services as your health needs change. Your electronic medical records are available when needed to all Anchor providers for seamless continuity of care, including additional specialty services and ancillary testing results. (Laboratory and Radiology Tests, Cardiac Imaging, Physical Therapy, etc.)

- Can treat acute illness quickly.
- Promotes wellness through disease management education including prescription management and education.
- Will be familiar with the physical, psychological and lifestyle factors that affect the health of you and your family.
- Can send you to an Anchor specialist and coordinate your ancillary care – lab work, radiology testing physical therapy and diabetes education.
